Thank you for your interest in the BODY-ALINE exercise machine. If you have Health Insurance and/or Medicare Part B, you may be eligible to receive full or partial reimbursement on the purchase price of your BODY-ALINE exercise machine. This reimbursement will come in the form of a check to you from your insurance provider(s).

Beyond the Curve L.L.C. (the creators of the BODY-ALINE) is a Nationally Approved Supplier of Durable Medical Equipment. This allows us to file a claim, on your behalf, to your insurance company for the purchase of durable medical equipment.

Here’s how the process works:

STEP 1: You must set an appointment with your physician concerning a prescription for the BODY-ALINE for you medical needs.

STEP 2: After setting the appointment, you must contact BODY-ALINE at 877-265-2225 and provide us with your personal contact information and your physician’s contact information, along with the date & time of your appointment. At this time, BODY-ALINE will then call your physician and provide them details of the product for your upcoming appointment.

STEP 3: After meeting with your physician, you must obtain a detailed prescription for your BODY-ALINE. The prescription must include the physician’s full name and contact information, date of the prescription, a diagnosis, and it must specify the BODY-ALINE by name.

STEP 4: Once you obtain the prescription, you must then provide the following to us

(via fax, mail, e-mail):

A photocopy of your prescription for the BODY-ALINE.
Your Primary Insurance Beneficiary Identification Number.
Your Secondary Insurance Beneficiary Identification Number (if applicable).

STEP 5: Once we receive the above items, we will then contact you to arrange payment for your BODY-ALINE. You may choose any one of the following payment options:

1) Sign up for the Totally Free 30-Day Trial: If you keep it past 30 days, then 7 monthly payments of $49.99 w/Free Shipping (TOTAL = $349.93). Or, if you keep it past 30 days, you can pay in full with 1 payment of $349.93 w/Free Shipping.

2) Monthly Payment Plan: 7 x $49.99 w/Free Shipping (TOTAL = $349.93).

3) SAVE $50.00 & Pay in Full: 1 payment of $299.93 w/Free Shipping.

STEP 6: Once we receive payment in full, we will then submit a claim on your behalf to your insurance provider(s). If you are on the payment plan, your claim will be submitted when the product is paid for in full.

All claims are handled by your insurance company(s) on a case-by-case basis. By submitting a claim on your behalf, we cannot guarantee you will receive coverage. Regardless of the outcome of your Medicare &/or Health Insurance reimbursement claim, you are ultimately responsible for the total cost of the machine.

If you wish to start the claim filing process today, insurance regulations do not allow you to place an order until you have a physician’s prescription. However, if you wish to forego the claim filing process, we can place your order today - without a prescription.
